Normally, our immune system manages small amounts of bacteria that constantly move through the body. But when a larger void develops around a failing root canal seal, bacteria can multiply and form a permanent “habitat.” This infection may go unnoticed for years—until it finally overwhelms the body’s defenses. At that point three things may occur from the failing root canal:

Abscess formation – swelling in the face or gum tissue, pain, fever, and an abscess visible on an X-ray.
sinus tract – the infection drains through a small open channel, leaving a constant pus drainage with little or no swelling or pain. Some patients notice a sulfur or metallic taste.
Pimple-like drainage cycle (gum boil)– a recurring pimple on the gum that fills with pus, pops, and drains repeatedly. Its basically a sinus tract that is closing filling as a pus cyst then discharging/ popping.

The gum tissue is gently lifted to expose the bone, and a small opening is made through the bone to reach the infected root tip.

Step 2 – Cleaning the Infection
Granulation tissue (infected or inflamed tissue) around the root is carefully removed.

Step 3 – Removing the Root Tip
The very end of the root is trimmed off, and a small preparation is made in the tip to hold a sealing material.

Step 4 – Sealing the Root End
The tip is sealed with a durable filling material, often MTA (a biocompatible cement) or sometimes amalgam, to prevent bacteria from re-entering.

Step 5 – Bone Graft
The cavity is filled with bone graft (synthetic hydroxyapatite), the gum tissue is repositioned and sutured, and healing begins. Patients typically take pain relievers, anti-inflammatories, and antibiotics for two weeks. with full bone healing expected over about 6 months.

Insurance companies raise premiums every year , yet lower reimbursement rates for dentists—forcing providers to work harder for less. This has nothing to do with improving patient care, and everything to do with maximizing corporate profits .

They’ve built a monopolistic grip over dentistry , controlling over 150 million Americans through restrictive PPO plans. Here's how it works:

�� The Insurance Playbook:

Step 1 : Secure a region by flooding it with in-network contracts (DSOs, chains, and group practices).
Step 2 : Funnel the majority of patients (about 60% of the market ) into these network providers .
Step 3 : Once enough providers are locked in, they lower the UCRs (Usual, Customary, and Reasonable fees).
Step 4 : Dentists are now “preferred providers” in name only—forced to work at deep discounts with little say.

Corporate dental chains, largely run by non-dentists, are rewarded for one thing: compliance. As long as they follow insurance company rules, they keep getting patients—while reimbursements keep dropping. Lower payouts mean higher profits for insurance companies, but worse care for patients.

Dental insurance Network market control:

Insurance companies set artificially low UCR (Usual, Customary, and Reasonable) fees, particularly in areas with heavy DSO contracted network presence. Let’s explore what causes supernumerary teeth, why they occur, and why removing them is often the best option.

What Is a Supernumerary Tooth?

A supernumerary tooth is an extra tooth that forms in addition to the normal set. These teeth develop from additional tooth buds during early development and can appear anywhere in the dental arches.

They are most commonly found in the upper jaw , especially:

Between the two front teeth (called a mesiodens )
Behind the third molars , sometimes referred to as a fourth molar

How Common Are Supernumerary Teeth?

Affects less than 1% of the population
More common in men than women
90% form in the upper jaw (maxilla)
Rarely appear in baby teeth (primary dentition)

What Causes Supernumerary Teeth?

The cause is often genetic . In many cases, it's inherited as an autosomal dominant trait , which means just one copy of the gene from a parent can cause the condition.

We have 22 autosomes and 1 sex chromosome from each parent = 46 total in one human. The gene or allele for the supernumerary tooth is located on the autosome. If one parent has it the offspring has a 50% to 100% chance or greater expressing it.

The one parent may be dominant and recessive for the allele from his parents in this case 50% chance of offspring expressing this trait.

If the one parent is dominant and dominant for the gene then it’s 100%.

Sperm (male) carries 22 autosomes + X or Y sex chromosome eggs (female) are 22 + X or X. Genetic information is always ½’d at fertilization then combined as the fertilized egg-embryo to become 44xy (male) =46 or 44 xx (female)=46!

Why Should Supernumerary Teeth Be Removed?

Even if they don’t erupt, supernumerary teeth can lead to serious problems:

1. Cyst Formation

Unerupted teeth can become trapped in fluid-filled sacs (called dentigerous or follicular cysts )
These cysts may expand causing erosion of nearby bone and roots

2. Tooth and Bone Resorption

Pressure from cysts or the extra tooth can cause loss of bone or resorption of adjacent teeth

3. Tumor Risk

Rare but serious: some cysts may transform into odontogenic tumors like:
Ameloblastoma
Odontogenic carcinoma

Luxate simply means displaced or dislodged from original position.

If the tooth is intruded or pushed up in it's socket (pic 1), It's best to avoid chewing or putting any pressure on this tooth allowing it to rebound back up. In many cases that's all that needs to be done. The case is watched over 6-8 weeks.

if the tooth is knocked right or left but not completely out (pic 2), with firm directed pressure push it back in the right direction. Minor shifts usually heal on their own with careful avoidance (no chewing). If its a major shift, Get to a dentist so he can aid in reseting and ridgedly splinting it. The case is watched over 6-8 weeks. if the tooth is firm then the fixation splint (heavy bonded wire) can be removed.

If worsening pain occurs with either intrusion or luxation, this is indicative of an irreversible pulpitis (nerve damage) root canal treatment will be needed. If the tooth is someplace shy of it's original position after 6-8 weeks then orthodontics will be needed to guide the tooth back to correct it's alignment.

A chipped tooth can be caused by stress cracking or just a sharp blow to the tooth. As long as it's minor like in this picture it can be fixed with composite. If it goes into the nerve chamber of the tooth it will require a root canal and crown.

Chipped teeth have symptoms of increased temporary sensitivity to air, hot cold fluids, or any that's chewed into the area. It will be a sharp pain that quickly (within a second goes away).

The reason for this is dentinal sensitivity. When the outer enamel layer is chipped off into the dentinal layer it exposes the open tubules of dentin that communicate with the nerve directly.

Is it painful to get a root canal?

The procedure itself is not painful because it's done under local anesthetic. It can take anywhere from 30 minutes to 2 hours. In some cases there is no pain after the anesthetic wears off. If pain does occur, it usually builds over the days that follow. Typically 4-5 days out can be the worst of it. The pain is due to inflammation at the base of the tooth. During the procedure debri from inside the tooth is cleaned out but some gets flushed through the tiny orifi at the end of the root. Bleach is used to clean the inside of the tooth but some of this gets flushed out causing a chemical burn which leads to inflammation pain.

Relief happens as the body clears the inflammation and the area heals. Pain (dull ache) can linger out 4-6 weeks but it's uncommon. Most pain will reside with in the first two. Pain medications, antibiotics, and anti-inflammatories help during this period.

The tooth should settle down and feel like the rest and be comfortable to chew on. However you should never chew anything hard on root canaled tooth until the finial crown in put on. Although the tooth may feel comfortable you need to follow up quick with the crown or else it could decay to a point it cannot be restored.

Implants date back to ancient times. Some of the earliest examples are from the Mayan Indian civilization in 600AD in Mexico.

The road that lead to modern implants was first stumbled on by accident when Ingvar Branemark was conducting in vivo studies of bone marrow cells in his lab in Sweden.

He surgically placed titanium oculars in the tibia and fibula bones of rabbits to microscopically observe the living cells inside bone marrow. At the end of the studies, the titanium oculars were to be removed. However, it was noted that the oculars had become fused to the bone through a process known as Osseointegration.

This accidental finding traveled through the medical community and led to breakthroughs in artificial limb replacements in humans. His findings were also disseminated through the dental community and led to research in replacement teeth in the form of titanium implants.

The first implants were placed by Dr. Branemark in an edentulous Swede by the name of Gosta Larsson in 1965. Gosta Larsson lived to 2006 using the implants placed and supported prosthetics for over 40 years!

Since that time, titanium implants have gone under much testing and redesign, which brings us to ever-evolving shapes and sizes of modern-day implants!

Keeping your teeth and gums healthy is incredibly important, especially when the cost of bad teeth could impact your health. With gum disease on the rise, studies have shown that keeping a healthy routine with your oral hygiene could help prevent your risk of heart disease.

Fortunately, there are a few ways to keep your teeth strong and healthy just by doing the following:

1. Brush twice a day and floss daily. Gum disease and tooth decay remain big problems — and not just for older people. Three-fourths of teenagers have gums that bleed, according to the ADHA. Along with the basic advice, remember:

Toothbrushes should be changed 3 to 4 times a year.
Teenagers with braces may need to use special toothbrushes and other oral hygiene tools to brush their teeth. Talk to your dentist or orthodontist.
Older people with arthritis or other problems may have trouble holding a toothbrush or using floss. Some people find it easier to use an electric toothbrush. Others simply put a bicycle grip or foam tube over the handle of a regular toothbrush to make it easier to hold.

2. Make an appointment. Most experts recommend a dental check-up every 6 months — more often if you have problems like gum disease. During a routine exam, your dentist or dental hygienist removes plaque build-up that you can’t brush or floss away and look for signs of decay.

3. Rinse or chew gum after meals. In addition to brushing and flossing, rinsing your mouth with an antibacterial rinse can help prevent decay and gum problems. Chewing sugar-free gum after a meal can also protect by increasing saliva flow, which naturally washes bacteria away and neutralizes acid.

4. Avoid sugary foods. When bacteria in the mouth break down simple sugars, they produce acids that can erode tooth enamel, opening the door to decay. “Sugary drinks, including soft drinks and fruit drinks, pose a special threat because people tend to sip them, raising acid levels over a long period of time,” says Steven E. Schonfeld, DDS, Ph.D., a dentist in private practice and spokesperson for the American Dental Association. “Carbonated drinks may make matters worse since carbonation also increases acidity.” Sticky candies are another culprit because they linger on teeth surfaces.

5. Don’t smoke or use smokeless tobacco. Tobacco stains teeth and significantly increases the risk of gum disease and oral cancer. If you smoke or use chewing tobacco, consider quitting. Counsel your kids not to start.
